Please provide a valid reason for requesting a conflict.

When requesting a conflict for a midterm exam,  if it is a conflict with another course, specify the course name/number (e.g.  Math 213).

When requesting a conflict for the final exam please list course name/number date/time for ALL of your final exams (e.g. Math 213  12/13 9am-11am).

If you missed an exam due to sickness, please obtain a note from the Emergency Dean. We will need to keep a copy of this note.

For any other reason, please be specific why you are making the request.  ("I'm very busy this week.", of course is not a valid reason.)
